Office visits are by referral and appointment only. We will work with you to reserve an appointment time convenient for you, so we require that any changes or cancellations be made at least 24 hours in advance. You will be charged a $50 fee if your appointment has not been rescheduled or cancelled in advance.
New patients are required to arrive 30 minutes before their appointment; and follow-up patients are required to arrive 15 minutes before their appointment. Patients may need to reschedule their appointments if they fail to comply with these requirements.
Monday through Friday from 8:00 AM to 5:00 PM

Your calls will be answered by our office staff; the information you provide will be conveyed to our providers. We will return your call with specific instructions by our medical assistants or by our providers during normal business hours.
Requests for prescription refills are handled during regular office hours and will be filled during normal business hours. Please have the name and telephone number of your pharmacy available when calling. We encourage you to check with your pharmacy before contacting us as refills may still be available on your current prescription. Please allow 72 hours for any refill requests.
We are participating providers with most commercial insurance companies and networks as well as Medicare and Tricare. We do not participate with Medicaid.
Your medical records are confidential. If you sign a release for them, we will gladly forward them to your primary care physician. For additional copies to other physicians, government agencies, insurance companies or your personal use, you will be charged for this service.
